Our Matching Dollar Program invites the applicant and their family, friends and associates, to make a donation to us on behalf of the patient.  We will match up to $1,500 of these donations and apply the combined funds toward a maximum of $3,000. The Matching Dollar Program is subject to the availability of funds.


DEADLINE TO RECEIVE CLAIMS AND DONATIONS:  The program covers date of service from November 1, 2017 thru October 31, 2018. The new closing date for us to receive claims is December 1, 2018 The deadline for us to receive donations is December 1, 2018.

FUNDRAISING LIMIT:  At any time during the program, you are only authorized to make or raise donations for half the amount in approved claims. Therefore, we set your fundraising limit with your first approved claim, and increase it as each additional claim is approved till $1,500 in claims are met.  No refunds are given for donated and matched funds at any time.  Consequently,  we must be sure your donations can be applied to your claims first.  Otherwise, if you raise donations before your claims are approved you are at risk of losing these funds if they cannot be applied to the claims between now and the experation date.

EXCEEDING THE FUNDRAISING LIMIT:  We realize that once the word is out and our flyers have been distributed, you have very little control over the amount of donations that will be sent in.  As a result, these donations may exceed the fundraising limit of $1,500.  Since we can only match up to $1,500 in donations per program, any excess donations will remain unmatched.
